The office is located at the rear of the building. Once you turn into the office park, make the second left and the building will be the second building on your left. There is plenty of parking available for your convivence.

What are your office Policies?

 
VeriFi Charlotte is pleased to serve Charlotte and the surrounding areas. We ensure thorough cleaning and sanitation of the office in between clients and each evening upon close of business to ensure the safety and health of staff and clients. We do ask that you wear a mask while in office. If you have experienced flu-like symptoms in the past 10 days, we ask that you reschedule your appointment. There may be times when we are extremely busy, in those cases you may be asked to call in once you have arrived at your appointment and wait in the car until you are asked to come into the office.
 
 

Appoinment Policy?

 
Please be sure to book your appointment in advance when possible. We do accept Walk-in's but to ensure that you are seen in a timely manner appointments are strongly recommended. A non-Refundable deposit is required to secure all appointments! Payment is due in full prior to any testing or screenings.
 
 

What do I need to bring with me to the appointment?

 
Please bring Signed Consent Forms, Covid Questionnaire, Authorization form from Employer (if Applicable) and unexpired Identification. Please also bring your form of payment. We kindly accept many forms of payment such as Cash, Visa, Mastercard, Amex and Apple Pay.

You may use the link below to access and print consent forms to bring with you to your appointment. Both the Consent form and Covid Questionnaire are required prior to being seen.


 
 

Can I bring someone with me to my appointment?

 
To ensure the safety and security of staff and clients, only those being tested are allowed into the lab and testing area. We ask that you do not bring anyone with you to your appointment. No group appointments will be allowed. Children under 12 are not permitted in lab and testing area unless being served.
 

How early should I arrive to my appointment?

 
Please arrive at least 10 minutes prior to your appointment time to ensure proper paperwork is completed prior to testing. Consent forms are also available online for you to print and bring with you to the appointment.
 

What if I am running Late?

 
We ask that all clients arrive at least 10 minutes before scheduled appointment time. If you are running late your appointment will need to be rescheduled. We will work with you to schedule the next available appointment time available!
 

What if I need to cancel my appointment?

 
We require a 24-hour cancelation notice for all testing services. If appointments are not canceled within 24 hours there will be a $25 cancelation fee. Any deposits paid are non-refundable.
 

Do you Accept Insurnace?

 
Currently, VeriFi Charlotte does not accept medical insurance. In some cases, your lab test and its payment may be accepted and reimbursed by your insurance company in accordance with the terms of your policy. Please check with your insurance company. Cash and all major credit cards are accepted.
 

Refund Policy?

 
All testing and screening services are Non-refundable.
 

Shipping Policy?

 
Shipments are processed within 48 hours of order confirmation. Please note orders may take 7-10 business days from the date of shipment to arrive. To ensure that your order is  processed correctly please make sure that your address is entered correctly  and includes all relevant information such as abbreviation's, street numbers and letters. Apartment numbers and letters.
